Please fax a copy of patient demographics, copy of insurance card, recent chart notes pertaining to the visit, plus any lab and/or pathology reports.
For Mohs Micrographic Surgery referrals: A pathology report is required and a photo if available.

Pathology
Together, our two professional pathology labs process specimens weekly for all of our locations. Our team of board-certified dermatopathologists provides timely, accurate diagnostic interpretation and clinician support for skin, soft tissue and nail disease. They remain in close cooperation with our clinical staff to maintain clear and accurate communication and timely results for quality patient care.

The Salem, Oregon lab is CLIA-certified (Clinical Laboratory Improvement Amendments), while the Mill Creek, Washington lab is certified by the College of American Pathologists (CAP).
